


Our lives are filled with forms—forms for taxes, forms for school, forms for restaurants, forms for offices. Some of the most important forms a person fills out are at medical facilities, those forms are similar from facility to facility, and even upon receiving treatment a patient is bombarded with questions. We live in a world where a face can unlock a device, a phone number shares a drink order, and a social security number holds tax information, yet medical facility forms are for the most part unchanged. INTAKE is a visual commentary of medical intake forms and how this necessary process has created inefficiencies in medical care. For my BFA Capstone project, I spent a year studying and researching how our intake processes, specifically in hospitals and other medical facilities, could be improved. While my final project wasn't a solution to the problem, it opened my eyes to different pain points and many possibilities that could improve intake forms.
